What causes a red rash and pain in the penis after sex?

Hello doctor,

I have a red rash on the shaft of my penis. The semen is clear few days after sex. It is painful at the tip as well. We had sex for three hours. I am not sure if that caused the rash or if it is a friction due to a tight condom. A day later, I noticed this swollen rash.

I am attaching a picture. I do not have any itching or burning. I am not sure if my partner had any problem. I had something similar in the past, but that was due to friction.

From the picture that you sent, I can appreciate the redness in the top third of the penis (attachment removed to protect patient identity). There seem to be three possibilities in this case: Contact dermatitis or eczema from the condom or lubricant or any other foreign substance you used during sex. Candidial Infection from the partner. Herpes, but that is very painful, so less likely in your case.

Usually, there is a simple test of the discharge, which can give you a conclusive answer. But, if that is not possible, I suggest you apply a cream containing Miconazole and Hydrocortisone, twice a day for three days. If Fluconazole is available in your country, then you can take a single tablet of Fluconazole 150 mg and daily oral Cetirizine. Do it for three days and revert to me with the status. If you notice any unfavorable developments, you can report earlier too.
