Hello doctor,

I have been advised to undergo a root canal on my second premolar. I have all my X-rays and I would like to know if this would be required or not. I have been experiencing pain in my jaw and upper teeth for one month. Currently, I am taking Frisium, Lamotrigine, and Keppra. Earlier I was on penicillin-based antibiotics.

I understand your concern. How long you have been experiencing the symptoms of pain. As the filling in your second premolar is quite close to the nerve, it can be the main reason. Let me know which antibiotic you are taking currently and when this filling was done on your second premolar. Was it long back?

Thank you for the reply.

Currently, I am not on any antibiotics but I was on penicillin-based antibiotics until last week. I cannot remember when the filling was done but I think it was a few years ago. I have been having the symptoms related to this tooth for one month only.

I do not think RCT (root canal treatment) is required. Do you have severe pain or just moderate pain in that tooth now? When a filling is close to the nerve and it exceeds that pressure, temporary pain will subside in a few days once you take the antibiotic course. Maybe you should just wait and watch a few more days after taking this antibiotic prophylaxis which I am prescribing now. Take Amoxicillin 500 mg twice daily for three days along with another antibiotic which I would like to give. Do not take any other medications. Please confirm your age for me to prescribe the other medication. If this does not work even after a few days and the sensitivity or pain persists, then you would need to get the infection resolved by endodontic or root canal treatment.

Kindly consult a specialist, talk to them, and take medications with their consent.

Thank you for the reply.

I have been on both of these antibiotics during the last month and after a few days of stopping them, the infection was back again. So I think it is better to get the root canal done.

RCT (root canal treatment) could be advocated, but it can also fail as you will need to adjust to it post-operatively. I wanted to know your medical history so that you can prevent this infection from relapsing by just following proper antibiotic therapy. The synergistic action of antibiotics would act the same as RCT and many patients do not get it done until they have severe pain. So I would like to know if you have severe or tolerable pain and your medical history.
