Patient's Query

Hi doctor,

I have had various symptoms throughout the week. I am a little worried as I have mild pain in my penis. I experience soreness and tenderness to pressure. However, I can walk, and I go to work every day. But I feel something abnormal. A day before the weekend, I had relaxed sex in the morning and long and vigorous sex in the afternoon. I did not ejaculate as I had to hold it various times, so I stopped it from coming two times. The next day, I felt a mild tenderness but did not pay attention, and I again had normal sex at night. But the following day, the soreness and tenderness were severe, although I still had vigorous sex in the morning, and I had normal sex at night. Now I feel soreness and tenderness while touching, more on the first inch of the tip (head). And I feel some discomfort. However, the pain is mild and dull. In addition, I have a few questions, is this pain normal? Or is something wrong with me? Do I need to take any medications for this? Is this soreness? Did I fracture my penis? Can this be an infection?

I doubt it is an STD. I had it in the past, and it took around three to four days for the symptoms, like a burning sensation while urination, to develop. However, the next day after these events, I started having this pain, so I think it was early for the symptoms to appear. Also, I go out often with someone so that anything can happen. But I still think it is too early for those symptoms to occur. Is it normal to have soreness and tenderness on the penis after a weekend of sexual intercourse?

Please advice.

Hi,

Welcome to icliniq.com.

I went through your query and understood your concern. It seems like soreness. STDs (sexually transmitted diseases) take at least four to five days to develop, and there would be the formation of rash, ulceration, or burning sensation during urination. So, I would not worry right now. And, I suggest you to apply some moisturizer if there is severe soreness. And, if the intercourse was unprotected, and if the symptoms continue, you can get yourself tested. If simple creams do not work, I suggest you to apply Lignocaine gel over the sore part in a small amount. It is a topical anesthetic cream that numbs the skin for about half an hour. Kindly consult a specialist, talk with them and take the medications with their consent.

I hope this was helpful.

Kindly follow up if you have further queries.

Patient's Query

Hi doctor,

Thank you for your reply.

It is not a burning sensation. Instead, it is a dull pain, like tenderness, if you apply pressure to it. And if I apply pressure, I feel pain in the first inch of the penis, exactly where the circumcision ends. This started the next day after the first day of my sexual activity. And on the next day, I started feeling soreness, but since it was mild, I continued having sex. Then two days after the first encounter, the soreness was severe, but since it was the end of the weekend, I had sex twice, and the soreness was still there. However, when I have sex, I do not feel uncomfortable. Now three days after the first event, the soreness and tenderness have become severe. Also, I have pain; on a scale of 1 to 10, the pain is around 3.

I took an Advil, but it was not beneficial. I could take the STD tests, but I think the symptoms started early. It is not even in the incubation phase, I think; also, I used protection every time. And if I have bruised or injured my penis, how many days will I have the pain? So should I abstain from sex and masturbation? In addition, I think sexual abuse created this issue. Also, I am afraid it could be something else. In addition, I believe it is not an STD since the pain started early, and even if she is a friend I used to go out with, I do not think she is like that. However, I still used protection every time.

In addition, I have HSV (herpes simplex virus) type 1 on my genitals, but I have not had an outbreak in 10 to 11 years. And right now, I do not see anything that could suggest penile sores. And generally, I do not remember that I used to have this dull pain when having an outbreak; it was just sores that triggered a burning sensation.

Please give your suggestions.

Hi,

Welcome back to icliniq.com.

Yes, STD is practically not possible if you have used protection and also consider the early onset. However, it is primarily due to 'overuse,' so I suggest you take rest as this will help you recover. There could also be minor bends and tiny cracks in the inside coverings, which would heal in about a week. And if the tenderness lasts for more than a week, I suggest you to consult your doctor. So for the time being, I suggest you to take an over-the-counter anti-inflammatory medicine like Brufen (Ibuprofen), and that will help. Kindly consult a specialist, talk with them and take the medications with their consent.

I hope this was helpful.

Take care.

Patient's Query

Hi doctor,

Thank you for your reply.

Today morning I developed symptoms like discomfort and pain but it is manageable. Also, when I pass urine, it does not hurt, and when it is stiff, there is no pain. I asked my partner about her history, and she said she has not been with anyone. However, I used protection every time. Although because of the early onset, I doubt if it is STD. Also, I feel pain in the inside of the penis 1 inch from the tip of that whole area but nothing in the base of the penis. Do you think it is an inflamattion? Or a crack in the inner part of the penis?

Please give your suggestions.

Hi,

Welcome back to icliniq.com.

Yes, minor cracks can occur in the coverings over the erectile tissue of the penis. This usually heals on its own, and you should be fine in about a week. However, if the problem persists, I suggest you to consult your doctor. Till then, take rest.

I hope this has helped you.

Thank you.

Patient's Query

Hi doctor,

Thank you for your suggestions.

The symptoms have not aggravated, and sometimes I forget I have an issue. But when I think of it more, I become anxious. But I still feel some discomfort, and when I press, there is mild pain, and I feel like I want to pee. In addition, I have not masturbated, but I usually get aroused and have an erection when I think or speak with a girl. When I went to the toilet to empty my bowel and passed urine, I saw ejaculation-like discharge in the pee. Is that normal? Then I tried to see if there was something in my penis, and nothing was abnormal. I again passed urine later, and it was normal. However, I felt a tingling sensation in the same area and there was mild pain. I have no burning sensation or discoloration while passing urine. I have peed four times, and I have not seen anything abnormal. What do you think this could be?

Please answer.

Hi,

Welcome back to icliniq.com.

So I guess things are getting back to normal. And yes, sometimes, when we strain while passing stools, some semen comes through the urethra, which is normal. So do not keep thinking about penis soreness.

I hope this has helped you.

Thank you.

Patient's Query

Hi doctor,

Thank you for your reply.

It has been one week since my penile soreness encounter. And I still feel the soreness. However, less severe than before. But I do not know if it is normal for the soreness to occur this long. However, my partner was normal when I felt this 1 to 2 days after I first thought about STD, I used protection all the time, and we did not have oral sex. And I keep thinking about this mild pain I have.

Please help me find a solution to this.

Hi,

Welcome back to icliniq.com.

I understand your concern.

The more you think about it, the longer it will persist.

I hope this has helped you.

Take care.

Patient's Query

Hi doctor,

Thank you for your advice.

I went to the toilet to empty my bowel, and when I finished and looked down, I noticed the semen again in the toilet. What to do?

Please help.

Hi,

Welcome back to icliniq.com.

Semen will be produced continuously till death, and maybe the amount decreases after ages 60 to 70. So whatever is produced must come out, either during the night or while straining. Sometimes we notice it, and sometimes, we do not. And sometimes, it flows out in the urine. So relax.

I hope this helps you.

Take care.

Patient's Query

Hi doctor,

Thank you for your reply.

But it looks as if I have ejaculated. Is it normal? I searched on the internet about it, and it suggested conditions like prostatitis, cancer, and STDs. I become nervous and have the urge to pee. I had not seen a leakage of semen when I urinated, and now it has occurred twice. It is probably because I am too nervous or something is wrong with me.

Please help.

Thank you.

Hi,

Welcome back to icliniq.com.

The volume of ejaculation can vary depending on the frequency of ejaculation. Also, if some semen comes out in the urine, the upcoming ejaculation can be less. However, things will become normal in a few days, so do not worry too much.

I hope this was helpful.

Take care.

Patient's Query

Hi doctor,

Thank you for your reply.

I have not seen any ejaculation while passing urine, and the pain is very mild. I feel a burning sensation on the top of the penis head but not the tip. However, my urine is normal, and there is no burning sensation while passing urine. Also, the pain has reduced, and when I do not think about passing urine, I do not feel the urge to urinate. However, if I think I have a mild urination sensation, I get the urge to pass urine. Also, I sleep well and do not pass urine at night. The burning sensation on the skin may have been caused due to trimming that area. When I trimmed it a week ago, I rubbed it while walking, and because of that, I think I may have gotten the scratches. I do not know if I am going crazy or too anxious thinking I might have something. Although I do not have any issues other than the side sensitivity and scratches, I am still worried.

Please give your suggestions.

Hi,

Welcome back to icliniq.com.

I feel you are overthinking, and it is not helping the situation. You need to tell yourself everything is alright, and this, too, shall pass. I suggest you to apply some Lignocaine (local anesthetic gel) gel over the burning part of the skin. It will numb that part for a while. Also, wear underwear if you are not wearing one. This prevents excess movements and scratches caused by clothes.

I hope this has helped you.

Thank you.

Patient's Query

Hi doctor,

Thank you for the reply.

Sometimes it feels like a burning sensation on the side of the penis. My other symptoms are normal. It is complicated to explain what I undergo. I have the urge to urinate, but I pass urine in small quantities. Do you think it can be a fungal infection? Or due to some allergic reactions? However, I used a condom to masturbate four nights ago. But I do not wake up in the night after this. But I still think it might be something else rather than STD. I take the tablet Zithromax 1000 mg. Can I continue taking it?

Please help.

Hi,

Welcome back to icliniq.com.

You may take the tablet Zithromax (Azithromycin), but I am not sure if it will help with the burning issue. Anyways, do not overthink. Forget what happened in the past. So, I suggest you to avoid intercourse or masturbation for 5 to 7 days if possible. And let us see how things work out.

I hope this helps.

Patient's Query

Thank you, doctor.

I took the medications yesterday, and the burning sensation has reduced. However, I have mild discomfort on the inside of the penis. This is a new symptom. Last night I had a sound sleep and tried not to think about these issues. But I cannot control myself from thinking about it. This morning I did not have pain, or it was less noticeable; I am not sure. And when I experienced the burning sensation, the pain was not noticeable too. However, I am using vaseline for the burning sensation.

Please help.

Hi,

Welcome back to icliniq.com.

That sounds good. So, let us wait for a week or so.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

I will let you know if something occurs. Do you think it could be an STD or a fungal infection?

Please help.

Hi,

Welcome back to icliniq.com.

Kindly do not worry about infection. Let us wait.

I hope this helps.

Patient's Query

Hello doctor,

It has been 16 days since the first encounter, and the burning sensation has probably gone, as I have not felt it lately. But I still feel some discomfort but do not see anything abnormal physically. Sometimes the urge to pass urine becomes frequent. And sometimes, I feel mild discomfort or pain in the penile head. However, I am not sure if there is something abnormal with me or if there is a penile head fracture because of the abuse. It has been seven days since I took the tablet Zithromax. If something was abnormal, I should be fine by today, right? Also, when do you recommend having unprotected sex?

Please answer.

Hi,

Welcome back to icliniq.com.

I would recommend using protection for a few more days. And handle your penis gently.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

For how many days should I use the protection? It has been eight days now since I took the tablet Zithromax. And also, what makes you think I should wait for a couple of days if you thought this was not an STD? Can you explain?

Please answer.

Hi,

Welcome back to icliniq.com.

Using protection for a few days ensures fewer chances of contracting STDs (sexually transmitted diseases). Second, if anything were to occur, we would know it was from a previous encounter. Also, using a condom could prevent skin soreness. Third, we still cannot say if it was STD. And we can never predict you will not get STD in the future. So it is better to be safe.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

But if I took the tablet Zithromax 1000 mg, would it cure all STDs? If so, when will I be free from STD? Should I take a test?

Please answer.

Hi,

Welcome back to icliniq.com.

I do not think it was STD in the first place. Anyway, the tablet Zithromax will not cause any harm. I only want you to be easy on the penis, avoid rough sex, and use protection, so the skin does not become sore.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for the reply.

But if it were an STD, would the tablet Zithromax help eliminate it? And from how many days of starting the Zithromax (Azithromycin) will I be clear of STD? And what causes the urge to urinate? Is it due to anxiety? What do you think?

Please answer.

Hi,

Welcome back to icliniq.com.

The tablet Zithromax (Azithromycin) acts immediately, so if any STD germs were there, they would be killed. It does not act on viruses like HSV (herpes simplex virus). The urge to urinate is most likely due to pain and anxiety.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for the reply.

I feel something is causing the discomfort. I still have mild pain in the penile head and a mild urge to pass urine. However, other symptoms have subsided. Is there a time frame for the pain and the urinary urgency to disappear? In addition, I sometimes feel my penis is cold; I do not know why. I have not felt this way before, so I am scared of this situation. Can I take the tablet Doxycycline for seven days? Is it mandatory to take it?

Please help.

Hi,

Welcome back to icliniq.com.

The more you keep thinking about the urge to urinate, the more it will continue. And do not take the tablet Doxycycline (tetracycline antibiotic). However, things have improved so far, so you should expect everything to be normal soon.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

Does it take too much time? Sometimes I feel mild pain in the penile head. Is there an over-the-counter medication for urinary urgency? Can there be an inflammation in the inner part of the penile head? My other symptoms have improved; however, the pain in the penile head is still there. Is it normal for it last this long? It has been almost 20 days.

Please help.

Hi,

Welcome back to icliniq.com.

It usually takes around 10 to 15 days to show improvement. And, yes, rough sex can cause minor cracks in the inner part, and this heals by inflammation. For urinary urgency, consuming citrus juices will help. Also, I suggest you to take Potassium Citrate syrup. Consume by mixing two teaspoons of the syrup in water, and take it thrice daily. If the urinary urge increases or persists, I suggest you consult your family doctor.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

If the urinary urge continues, what can the problem be? Is it an STD? Today morning eight hours after ejaculation, when I passed urine, there was no pain, but I felt a strange feeling in the penile head but not the tip. What do you think it could be?

Hi,

Welcome back to icliniq.com.

If the urinary urge continues, we can think of swelling in the prostate, which has to be checked by your doctor before giving medicines. However, if a mild urinary urge is there and does not worsen, then there is nothing to worry about.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

For some reason, this afternoon went for a beer, and I had no urinary urgency, and sometimes the urge is not even present. But the discomfort feeling on the head of the penis is still there. So my biggest concern is that do I have STD. However, I used protection, and then I took the tablet Zithromax. So I do not know the possibility of getting an STD. Or is this because I damaged or hurt the penis because of the rough and abusive masturbation.

Please help.

Hi,

Welcome back to icliniq.com.

Kindly forget about the STD right now. Let things heal completely.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

But it is not easy for me. But what do you think the chances are? If I used protection in addition to taking the tablet Zithromax, can I still get it?

Please answer.

Hi doctor,

Thak you for your reply.

The chances are almost nil.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

It is a big relief. So the inflammation, or the issue I have, is not contagious?

Please answer.

Hi,

Welcome back to icliniq.com.

Yes, it was due to overuse.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

That is good, so it is not contagious, right?

Please answer.

Hi,

Welcome back to icliniq.com.

No, it is not contagious.

I hope this helps.

Patient's Query

Hello doctor,

Thank you for your reply.

Everything has been the same. But today, I felt the same bothersome feeling in the penile head and mild urinary urgency from time to time. There were times when I did not encounter urinary urgency. I think I was distracted at that time. And I think it can occur because of my nervousness when I think that I have an STD and start getting the urge. I have not gotten the burning sensation after that. However, there is still mild pain, but if I touch it and try to feel it to see if it hurts, I feel a vibrating sensation. Also, when my underwear rubs on my penile skin, it becomes sensitive and tender. But I cannot say it has always been like this.

What could this be?

Hi,

Welcome back to icliniq.com.

You must ride it out and not think about STDs for a few days. Distract yourself by playing outdoor games or going to the gym.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

I had unprotected sex with my ex-wife, who has been in my house for the past four days. Can she be exposed? Or is she safe?

Please answer.

Hi,

Welcome back to icliniq.com.

She will not get any infection from you because you do not have an infection right now. But if you continue to have unprotected sex, each encounter increases the risk of getting an infection.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

Even with the discomfort in the head of my penis, this might be inflammation, but not an infection. That is what you are trying to say, correct? It has been almost a month since the first encounter.

Please answer.

Hi,

Welcome back to icliniq.com.

Yes, there is no infection.

I hope this helps.

Take care.
