Hello doctor,

I am 49 years old. I had itching in the urethra around three years ago. I never went to the doctor regarding it. This year I suffered from pain in the lower left abdomen. In addition, I have swelling and pain in the left testicle. I do not take any medication except Paracetamol. How do I get rid of this pain and swelling?

I do not have any pain or other problems while urinating. The flow is always the same. I can make the jet flow faster when I push, but I almost never do that. A few months ago, I had to get out of bed more often to urinate, and also, the last year, I had moments when I had to get out of bed more often. There were nights when I sometimes had to get out of bed three times, but they were rare. I can hold my urine for a long time and do not have to run to the toilet quickly. But I know holding your urine is bad for the bladder, so I do not do that anymore. There is no anal pain at the moment, but sometimes I feel light pressure. I do not feel any pain when I ejaculate. When I masturbate, the feeling in the abdomen and testicles goes away. The left testicle has been slightly swollen for a few days and feels uncomfortable. A few months ago, I grabbed my left testicle, and when I pressed it was painful. I have never had my prostate and blood sugar tested. I have been walking around with a urinary tract infection for years. I am otherwise healthy as far as I can judge, except for slightly high blood pressure, and I am a bit overweight. So I am not completely healthy anymore. I have never been to the doctor with a urinary tract infection.

I feel that you may have some age-related prostate enlargement along with some prostatitis. Kindly get some tests done which are, ultrasound KUB (kidney, ureter, and bladder), uroflowmetry, urine routine, and blood sugar levels. For the time being, please start these medications. We may add more once you have done the tests: tablet Cranpac D (cranberry extract and D-mannose) twice daily, and tablet Econorm (Saccharomyces Boulardii) 250 mg. Consult a specialist doctor, discuss with them, and take medications with their consent.
