Patient's Query

Hello doctor,

I had epididymitis. All other infections, including STD (sexually transmitted disease), are negative. I had two rounds of antibiotics, and the symptoms were not alleviated. Epididymitis eventually passed. However, a constant urge to urinate presented. Testicles and prostate scan are normal. There is no effect on Mirabegron and NSAIDs. The urge to urinate is getting worse. The feeling is not constant, however, it comes and goes during the day. There is no symptom during sleep. I feel a drop of urine is left in the urethra after urinating. It presents randomly at any point in the day and not usually in the morning. I had Bactrim (Trimethoprim and Sulfamethoxazole) for ten days, followed by Ciprofloxacin for the other ten days. Currently, I am taking Mirabegron - 50 mg once and Naproxen/Esomeprazole - 500 mg/20 mg twice daily.

My blood, urine, and kidney function tests are normal.

This is a typical presentation of prostatitis. Although all tests are normal and the prostate could appear well on ultrasound, it can still be inflammation and not necessarily an infection of the prostate. Therefore, treatment usually is by prolonged antibiotics. I suggest, Septran DS (Co-trimoxazole) twice daily for four weeks, tablet Tamsulosin - 0.4 mg once at night for 28 days, and tablet Celecoxib - 200 mg twice daily for four weeks.
