Hi doctor,

I am a 23 year old sexually active female in a monogamous (long distance, so intermittent sex) relationship. I have very severe chronic yeast infections one lasting three years requiring Diflucan every day for a month and every week for 6 weeks. I also suffer chronic UTI, yeast infections resulting from antibiotics. I was diagnosed with cytolytic vaginosis with all of the symptoms feeling like a urinary tract infection during urination, extreme itching, uncomfortable, excess discharge with an odor, and pain during and after sex. A gynecologist prescribed baking soda and water baths.

I read online about someone who filled gelatin capsules with baking soda and took one on day 1 and second on day 3 and it cleared the issue up completely.

I was wondering if, taking my medical history into account, this would be a good idea for me. I am seeing my boyfriend in a week and I would like this issue to be resolved.

I could understand your situation. You have been treated a lot but are unable to get rid of your issue. Cytolytic vaginitis is associated with the overgrowth of Lactobacillus and causes the lysis of vaginal epithelial cells. It is not responsive to antifungals. Cytolytic vaginitis could give pruritis, dyspareunia, and vulvar dysuria as you are facing. Vaginal pH disturbance is also observed in 4 to 4.5 cytolytic patients.

My advice;

  1. Go for complete regular treatment if you have any health issues like diabetes, thyroid, hypertension, anxiety, and stress.
  2. Maintain strict hygiene.
  3. Wear soft comfortable undergarments with sterile pads as you have been experiencing excessive discharge.
  4. You can have Sitz baths, and sit in warm water to reduce irritation.
  5. Use condoms to avoid sexually transmitted infections.
  6. Douching with Sodium bicarbonate solution or use Sodium bicarbonate suppository. Douche twice weekly for every two weeks.
  7. To prepare douche use 1 to 2 tablespoons of baking soda with four cups of warm water.
  8. For convenience one gelatin-coated capsule of baking soda intravaginally twice weekly for two weeks and can be repeated treatment after 3 to 4 weeks to maintain a vaginal environment.
