Hello doctor,

I have very sensitive skin and have been struggling with acne for several years. From the picture I have shared with you ironically, it is the best it has been in a while. On my cheek, I have blackheads, acne scarring, redness, and dull and tired skin. For me, this is my worst area. I also have redness and blackhead on my nose, sides of the forehead, around the mouth, and jawline. I find that my skin seems to have excess oil in some and then dry in others making it difficult to find a product that works for it. In the morning I currently use face wash and moisturizer. At night I use the same face wash. I am using the oil control every other day and I also use Epiduo gel (Adapalene and Benzoyl peroxide). When I have particularly dry skin, I use commercial deep moisture comfort balm. Also once or twice I use either energizing scrub or organic olive and aloe scrub for gentle facial peeling. Also about once a month I use a dead sea mud mask. For me, I think the main issue is the acne scarring and redness of my cheeks and my dull tired complexion. Kindly suggest.

I have read your query and understand your concern. Post-acne scars are of three types box scars, icepick (pinpoint scars), and rolling scars. Pinpoint scars are difficult to treat while box scars are easy. I would like to suggest the following:

1. No cream or tablet can help you out with scars.

2. Best technique is CO2 fractional laser plus subcision (a surgical procedure to remove acne scars). It takes about five to six sittings but results are better than any other procedure.

3. Derma roller plus subcision is another technique but less effective than above.

3. Fillers have a hiding effect that fills the pits and gives an impression of fullness but it is not permanent.

4. Chemical peels can be effective if scars are not much prominent.

These are techniques you can go through. All of them will take four to six sittings and do it for five to six months for better results.

For dull complexion:

1. Avoid direct sunlight exposure as much as you can. Wear sunglasses, and caps to cover the exposed parts of the body all the time.

2. Use sunscreen thrice a day like 10 o'clock, 12, and 3 o'clock all over your face.

3. Sunscreen SPF (sun protection factor) should be around 50.

4. Use anti-pigmentation cream over pigmented parts like Kojic acid (Glycolic acid), Hydroquinone (Capryloyl Glycine and Glycerin), or Depiwhite (Glyceryl Stearate and Cetearyl Alcohol) cream.

5. If tanning is deep and too much then the ideal treatment is glycolic acid peels. That requires sittings of six to eight peels

6. Never stop sunscreen even if tanning goes off.
