Hello doctor,

I am a 21-year-old female with a weight of 52 kg and a height of 5.3 feet. I have bowel movements every three days, but I do not feel the need; I have to force it out. Last year I had gone five days without any movement, and it was painful to poop. When I did, it was excruciating, and the blood came out fresh. It has happened thrice since then, not in the past nine months. Now I can feel the skin tags around my anus. I have never had absolute constipation, and vomiting, or abdominal distention either, and I have had no abdominal pain in this whole period. For a while in between, my constipation was altered with diarrhea. My paternal aunt had cancer at the age of 26. My father and some of my paternal cousins have constipation. At the same time, one of them had hemorrhoids too. I noticed blood last year twice or thrice, and it was around a tablespoon every time; it comes with the poop, and it was excruciating. I do not take much fiber, but I drink around 1.5-liters of water every day.

It seems to be a case of anal fissure, which is a break in the lining of the anal canal. This can occur at times of severe constipation. Constipation can sometimes happen because of the decreased amount of fiber in the diet, water intake, and lack of exercise, contributing to it. You do not seem to have an increased risk of colon cancers. I suggest you take following and update me about your symptoms in 3 days. I would then decide about the future course of treatment. Use Movicol sachet 17 g (stool softner) twice daily in a glass of water for five days. Then once at night for another seven days. Isabgol husk two tablespoons full once daily with a glass of water. Irrespective of your constipation. Apply Xylocaine (Lignocaine) 2% ointment in the anal canal using a nozzle. Apply every six hours for next three to five days. Let me know in three days how you perform.
