Hi doctor,

I am 24 years old. Four weeks back, I had double protection sex with a lady. During sex, the condom got slipped and broken. I took it out immediately within few seconds. I tested for viral markers on the same day and the test result came back negative. I am having sickness for a week. I have myalgias, leg pain, hand pain, sweating, and slight chills. I do not know whether the lady is HIV positive or not. Am I at risk of getting HIV? I am afraid and please clarify my doubts. Should I need to test it again?

I understand your concern. Viral markers will not show a true result when it is taken on the same day. Now you have some symptoms and it is better to get tested yourself. Even though you have withdrawn immediately, still, you are at risk of getting an infection. Do not hesitate and consult a doctor soon and do appropriate tests at the earliest. It can be any sexually transmitted disease (STD) and not HIV (human immunodeficiency virus) alone. For the majority of STD, you have much better drugs to treat the condition.

I have gone through the reports (attachment removed to protect patient identity). Your reports look good. You have a low platelet count and high TSH (thyroid-stimulating hormone) levels. These can be correlated clinically after seeing all the reports. Both do not specifically indicate an STD (sexually transmitted diseases). On considering the exposure history, you need to do the following tests. You need to do hepatitis B surface antigen and hepatitis C core antigen test (HBsAg and Hep C Ag), HIV (human immunodeficiency virus) RT-PCR (reverse transcription-polymerase chain reaction), syphilis and urine routine. Please mention whether you have a discharge from the penis and any sores, rashes, and lumps on the private parts. Please mention whether you have any swelling in the thigh region. Please mention whether you have any thyroid issues and any bleeding after minor injuries.

I do not have sores, lumps, and rashes on my private parts. I do not have swelling in the high thigh region and I do not have thyroid. I do not have bleeding after minor injuries. I got vaccinated for hepatitis B. I did tests two times. Both the results came negative for HBsAg and Hep C Ag. I do not have any discharge from the penis.

Vaccination with hepatitis B will not protect you from getting an infection but it prevents serious complications. TSH is in the high range. Kindly wait and repeat after six weeks. It is good to hear that you do not have any lesions on the private parts. It is better to do the tests suggested by me. They will confirm the presence or absence of the specific disease. You will get physiological relief. Otherwise, you will remain with fear.

Just mention syphilis serology to the laboratory technician and he will do it. You can have the symptoms due to simple viral infection or COVID-19 (severe acute respiratory syndrome coronavirus 2). As you have a history of exposure with a sex worker for a brief time and you have symptoms after three weeks, so I suggest you do these workups. I understand your concern. It is important to rule out the possibility of STD. I wish you to have a negative result. If any of them is positive, it is important to start treatment at the earliest to prevent further complications. You need to catch it before it goes out of control. Pre-exposure prophylaxis should be taken before exposure. You have crossed the time period of post-exposure prophylaxis which is started after you are exposed to the risk. Both these options are not suitable for you now. If you had a possibility, I would have recommended the same.
