Hello doctor,

Over the last few months, and years I have few issues with my genitals.

I started to see small red and white pimples which come and go. I consulted urologist, who examined and said it is a fungal infection. He prescribed me an ointment to apply for two weeks but suggested to go for circumcision for a permanent cure. Ointment Clotrimazole and Betamethasone dipropionate cream 45 mg. I then consulted a few other doctors and dermatologists. They said not to worry and said it will be alright. Over the past month or two, I have got symptoms of itching, pain, and red or white bumps on my penis head. My wife too now has an itchy vagina and has a white liquid coming out. I have been applying this cream lately but stopped after I read your response to the above thread, when I pull my foreskin I get pain. I have no pain while urinating, masturbating, or having intercourse. Sometimes I have a slight pain in the scrotum. We are very concerned about this and request your kind attention to this. From the pictures (images attached removed to protect the patient's identity), this is Candidal balanoposthitis. Fungal infection of the glans (tip of the penis) and prepuce (foreskin). I would like to know if you have tested your blood sugar levels because diabetes is the most common cause of recurrent nonhealing candidal infection.

Kindly stop using that combination cream, Betamethasone is a strong steroid, and it further aggravates fungal infection. Once you stop using the Betamethasone cream, there will be more itching, pain, and red, and white bumps. I suggest you use,

1. Clotrimazole 1% cream, apply to the affected area twice a day, after cleaning the area with lukewarm water and mild soap.

2. Take a capsule of Candiforce (Itraconazole) 200 mg, it is a single-day treatment. Take one capsule in the morning and one capsule at night after food.

Avoid unprotected sexual intercourse until your and your wife's infection has resolved. Your wife needs to be treated simultaneously because fungal infections are contagious, if not treated you may get an infection from her once you get cured. I hope I have cleared your doubts.

Thanks for the reply. I have not tested my sugar, will test it and let you know the same. I have a few questions:

1. Is the same set of treatments applicable to my wife too?

2. How long shall I apply the cream and can I apply it on the penile head too?

3. For my wife, what is the method in this case?

4. Is circumcision a long-term solution?

5. Should I buy any special soap for bathing the genitals?

6. At this moment for me, itching is not that much but for my wife, she has occasional itching throughout the day.

7. For sexual intercourse, should we only engage after treatment or can we use condoms?

We are concerned about how this came in the first place. Kindly guide me on the same.

You do not need any special soap, normal mild soap is sufficient. I suggest you use Clotrimazole cream for six to eight weeks, since you have used Betamethasone, it may take a little longer duration to heal. If the issue is recurrent, then circumcision is advised. The treatment for vaginal discharge in women is different, she has to take tablet Candispan (Beclometasone and Clotrimazole) 3 kit, which has four tablets, one tablet of Fluconazole (Microcrystalline cellulose, Dibasic calcium phosphate anhydrous, Povidone, Croscarmellose sodium), one tablet of Azithromycin (Croscarmellose sodium, Dibasic calcium phosphate anhydrous, Hypromellose, magnesium stearate) and two tablets of Secnidazole. This is also a single-day treatment, Azithromycin has to be taken in the morning before breakfast, and fluconazole after lunch, tablet Secnidazole (Secnidazole, Guar gum, Xanthan gum, and Chitosan) after dinner. Topical clotrimazole cream, apply twice a day after cleaning the vaginal area. For women, there are many causes of vaginal discharge, she has to be examined. She should also get her blood sugar levels tested. Consult your specialist doctor, discuss with them, and take medications with their consent. It is better to avoid sexual intercourse even with condoms, as it may cause friction and delay the healing process. I hope I have cleared all your doubts.

Medication for me:

1. Clotrimazole 1% cream

2. Capsule Candiforce (Itraconazole) 200 mg,

Usage, It is a single-day treatment. Take one capsule in the morning and one capsule at night after food.

Medication for my wife:

1. Candispan 3 kit.

Usage, Candispan 3 kit, has four tablets, one tablet Fluconazole, one tablet Azithromycin and two tablets of Secnidazole. This is also a single-day treatment, Azithromycin has to be taken in the morning before breakfast, and Fluconazole after lunch, Secnidazole after dinner.

My query is about Secnidazole, Does she need to take the two tablets at the night?

2. Topical Clotrimazole cream.

Questions:

1. For how long my wife needs to apply the cream?

2. Are these medicines available over the counter?

Your wife needs to take two Secnidazole tablets at night after food. Clotrimazole cream is for six to eight weeks. These medicines are prescription medicines, Clotrimazole is OTC (over-the-counter) but not a tablet. Candispan is the brand name, I have also mentioned the name of the medicine inside the kit, Azithromycin, Fluconazole, and Secnidazole.
