Hello doctor,

I am a 21-year-old individual who has been dealing with a severe bladder problem for nearly two years. I experience an urgent need to urinate approximately every ten minutes, and this frequency is not an exaggeration. Despite medical consultations, my parents have stopped providing support for my condition. I am constantly thirsty, but diabetes has been ruled out as the cause. One of the urologists I visited injected Botox into my bladder, but it seems to have made my symptoms worse. Presently, the inside of my vagina is extremely sore, feeling raw, and swollen, and causing a burning sensation regardless of urination. I have noticed a thin white discharge around the area. Both my bladder (or genital area) and the lower part of my abdomen are significantly painful.

Additionally, my stool appears to have a fungal-like substance and thick green mucus, and I have been experiencing constipation. The pain and discomfort I am enduring are overwhelming, and I am at a loss for what to do. I am even contemplating self-harm. Given the prolonged duration of my symptoms and their impact on my daily life and sleep, I would greatly appreciate any guidance or recommendations on potential causes, such as infections, UTIs, or STDs.

It is important to monitor your nighttime bathroom visits. Please keep a record of the frequency and volume of urination to determine any changes in output. Additionally, please provide the results of your urine analysis report. Have you been assessed for tuberculosis? Kindly share the results of your ultrasound KUB (kidneys, ureters, bladder) test, including the measurements of full bladder capacity and post-void residue. Furthermore, please share the findings of the cystoscopy procedure, especially any observations made during the injection of Botox.

If previous attempts, including Botox treatment, have not been effective, I recommend trying the above combination. If medications do not provide relief, urinary bladder augmentation can be considered, involving the use of an intestinal segment to increase the bladder's capacity.
