Patient's Query

Hi doctor,

I have been following the advice given to me for the past two weeks. While using the Clotrimazole or Fucidin cream, the redness on my glans has mildly reduced. However, when I stop applying the cream, the redness returns (although it is still mild). I have also been using Cetaphil cleanser twice daily, but I have noticed that after drying it with tissue, the glans become very dry the next day. Sometimes it is difficult to retract the foreskin, although I am able to do so. I have attached a picture of the dorsum, which appears slightly discolored and dry.

Please help.

Hi,

Welcome to icliniq.com

Thank you for your query,

It is Candidial Balanoposthitis which is a fungal infection that affects the glans penis and foreskin. Treatment involves the following use of antifungal medication and maintaining proper hygiene to prevent the spread of infection.

  1. As per the treatment plan, I recommend taking Tablet Fluconazole 150 mg once a day after food for three days to eliminate the fungal infection. Additionally, applying Clotrimazole cream at night and Micogel cream in the morning for seven days can help relieve symptoms like itching, redness, and inflammation.
  2. It is important to stop using any cleansers or harsh soaps in the affected area and only wash with lukewarm water. After washing, gently pat the area dry with a soft towel, avoid tissue paper, and avoid any friction as it can worsen the condition. Maintaining proper hygiene by keeping the area clean and dry is important as moisture can promote fungal growth.
  3. I suggest checking blood sugar levels to rule out any underlying diabetes, which can increase the risk of fungal infections. Wearing loose, comfortable, cotton inners and avoiding tight clothing can also prevent sweat buildup and aid in healing. It is important to avoid activities that cause sweating and maintain proper genital hygiene.
  4. Lastly, I suggest getting a urine routine or culture test done to rule out any possible urinary tract infections or other underlying conditions.

Thank you.

Patient's Query

Hello doctor,

Thank you for the reply.

I have a few questions regarding my treatment.

  1. Should I continue using Fucidin cream at noon?
  2. Is it good to use tissue paper to dry the area after using Cetaphil cleanser?
  3. Should I apply the cream all over the glans or only on the red area?
  4. I am also concerned about the discoloration on the upper surface of my glans and would you advice any guidance on how to manage this?

Please help.

Hi,

Welcome back to iclinq.com.

Discoloration of the penis can be caused by various factors, and in your case, it is due to a fungal infection caused by Candida albicans. The treatment plan should be aimed at eliminating the fungal infection to restore the normal color of the penis.

  1. You should stop using flucidin (Fluconazole) and avoid the use of tissue paper, friction, rubbing, and scratching in the affected area. These practices can irritate the skin and worsen the condition. It is also essential to avoid any injury to the glans as it can make the infection worse.
  2. I suggest applying the antifungal cream only to the affected area to target the fungal infection. Ensure to follow the instructions for applying the cream provided by your healthcare provider.
  3. In addition to the treatment plan, maintaining proper genital hygiene is also essential in preventing the recurrence of the infection. Washing the affected area with lukewarm water and drying it thoroughly afterward can help prevent the growth of the fungi.
  4. Please continue with the prescribed treatment plan and follow up with your healthcare provider to ensure complete resolution of the fungal infection and to prevent recurrence.

Thank you.
