Hello doctor,

In an initial test, I tested positive for HBsAg. Then I went on to do a liver function test and it was normal (no abnormality). What do you think about this?

Well, it seems that you have chronic Hepatitis B infection. But to decide whether to give treatment or not, I have to get some more information about the state of current infection through blood tests. Thereafter I can suggest you the further course of management.

It is not uncommon to have HbsAg positive for years before any damage to liver ensues. Thankfully your LFTs are normal, but again it has to be interpreted in light of some other tests.

You have to get:

1. Complete blood count.

2. Serum creatinine.

3. Alpha-fetoprotein level (AFP).

4. Fibroscan of the liver.

5. Ultrasound of liver.

6. HBeAg.

7. HBeAb.

8. Quantitative HBV, DNA, and PCR.

9. A fresh LFT report.

You get these tested in a week or so and once reports are available we can discuss the further treatment plan.

The Probable causes

Chronic hepatitis infection.

Regarding follow up

Follow up after investigations.

Is it possible that a patient will have chronic hepatitis B and yet have a normal liver function test results? I thought that with the presence of hepatitis B the liver enzymes will be elevated.

Yes, it is absolutely possible and we as hepatologists very often see this. I would wait for your results. Nowadays the liver societies are recommending treatment despite even effects on liver function tests the one obvious reason is that HBV (hepatitis B virus) is linked with liver tumors and developments of chronic liver disease in long term. I am sure we would have answers after seeing your reports.

Same symptoms don't mean you have the same problem. Consult a doctor now!
