Hello doctor,

I am an 18 year old female, and my blood pressure is 148/101 mmHg. I am not on any medication, and I want to change my doctor because my doctor is too far for me to travel. I cannot get an appointment for two weeks. Do I need to go to the hospital to get medication? I am really worried about it. I almost fainted at college the other day, and I could not feel my legs from the bottom of my toes. I have been feeling very dizzy and exhausted recently, even after sleeping for long.

Hello,

Welcome to icliniq.com.

  • Your blood pressure is borderline, and it is not very high. It is not an emergency to start medicine. Instead, you need to monitor it frequently. You being a young hypertensive, you will need an evaluation for the cause of high blood pressure. I suggest you get the following tests done:
  1. Kidney function test.
  2. Renal artery Doppler.
  3. Serum cortisol.
  4. 24-hour urinary metanephrines.
  • You should have a healthy lifestyle, and I suggest you try doing the following:
  1. Avoid fatty, oily, and high-calorie diet.
  2. Have a low salt diet.
  3. Monitor your blood pressure regularly, thrice a day for one week, then once or twice a week.
  4. If your BP is persistently more than 140/90 mmHg, then you should be on medicines for it like tablet Amlodipine 5 mg once a day.
  5. Regular exercises like brisk walking and jogging, according to your capacity for at least 30 minutes a day and five days a week.
  6. Avoid smoking and alcohol, if any.
  • For fainting, we need to find out the cause, were you fasting for long or dehydrated or stressed? If yes, then you need to avoid precipitating cause. Increase fluid intake. Otherwise, you will need ECG (electrocardiogram) to rule out any cardiac cause for fainting, especially if you had any palpitations.

Hope this helps you and get back if you have any doubts.
