Hello doctor,

I am a 22 year old male, I smoke weed and cigarettes every day, and do not eat food at the right time. So, I have gastric problems and I puke every day, from the past six months. What to do?

I have gone through your details, and I appreciate your concern.

As per the history, most likely you have severe gastritis with overlapping esophageal spasm and gastroesophageal reflux.

It is because the stomach acids regurgitate back into the esophagus, causing heartburn and severe burning sensations.

Dyspepsia can be a sign of gastroesophageal reflux disease. Dyspepsia can also be a sign of a stomach ulcer or gastritis, which is an inflammation of the lining of your stomach.

I suggest you get an upper gastrointestinal endoscopy, to rule out any erosion of esophageal mucosa or Barrett's esophagus or ulcers.

I suggest you try the following. Consult your specialist doctor, discuss with him or her and start taking the medicines after their consent.

  1. Try taking Nexium (Esomeprazole), initially take 40 mg twice daily, but if not relieved then the doses can be increased to 80 mg twice a day.
  2. Domperidone is very effective in treating reflux not controlled by PPI (proton-pump inhibitor), especially when combined with Rabeprazole or Esomeprazole. 30 mg slow release dose can be taken daily.
  3. Eat simple, natural, and organic food. Eat a lot of raw salads.
  4. Avoid alcohol or smoking.
  5. Do not consume aerated beverages or soda.
  6. Avoid meat, processed food, cheese, cream, milk, etc.
  7. Take a lot of yogurt.
  8. Drink at least eight cups of water every day.
  9. Avoid all fruits which are citrus, like orange, pineapple, etc.
  10. Drink a glass of cold milk daily half hour before meals. Milk being alkali, it neutralizes the stomach acids.

If your symptoms are not relieving, please discuss with your primary physician.

To be on the safe side, I would suggest getting an EKG (electrocardiography) done because sometimes atypical angina can present with same symptoms.
