Hello doctor,

My 17-year-old sister is experiencing extreme stomach ache with vomiting. Every time she is in her period, this happens, but it is not her period right now. She is not taking any medication now. Used tablet Buscopan earlier for acid reflux but already vomited. What can I do for now? What do you think it is? Please help.

I understand your concern.

Since she is having persistent vomiting and abdominal pain, I will suggest you take her to the hospital. Reason is whatever tablet she will take, she will vomit it out and the tablet would not work. she will need intravenous injections to resolve her symptoms. Once she recieves intravenous injections, and vomiting stops then you can give her tablets to control the situation.

So for now, I suggest she will need following injections:

1. Injection Pantop 40 mg IV (Pantoprazole 40 mg).

2. Injection Perinorm 10 mg IV (Metoclopramide 10 mg).

3. Injection Lopez 0.25 ml IV (Lorazepam 2 mg).

4. Injection Drotaverine one stat IV.

In case you are not able to take her to hospital, I suggest you following medications.

1. Tablet Ondem-MD (Ondansetron 8 mg), to be kept below the tongue. It will be absorbed immediately and she will not vomit out

2. Tablet Metoclopramide 10 mg thrice daily.

3. Tablet Pantoprazole 40 mg twice daily.

4. Tablet Drotin DS (Drotaverine 80 mg) one tablet twice daily.

5. Tablet Escitalopram 10 mg at night.

Once her symptoms are controlled, I suggest you take tests like ultrasound of abdomen, blood sugar, and kidney function tests to evaluate the cause of these symptoms.Whatever it is, let me assure you that it is completely managable with medications. Nothing to worry about that. Hope this helps. Kindly revert back in case of further queries.
