Hi doctor,

I have been dealing with irritation on the head of my penis for almost a year now. It started as redness and then some small bumps developed, which led to a tight foreskin that hurt during urination and on retracting the foreskin. I consulted multiple doctors and dermatologists but nobody could find out the reason. Recently, I have found some blackish sores on the inside of my urethra and was wondering if it could be related to my previous problem, or if it is of any concern. There is no pain or pus discharge. However, I have noticed the sores look inflamed possibly. The redness and bumps on the penis seem to flare up after ejaculation or when dehydrated. I had used topical steroids, anti-bacterial, anti-fungal, and anti-yeast ointments for my penis issue. I also went for a swab test of the glans of the penis but the results were clear.

You have mentioned that you have pain while retracting the prepuce (foreskin), this can be a sign of an infection called balanitis. If you have already applied all different creams and still the problem persists then circumcision might be the option, that you will need to discuss with your treating urologist. About the sores inside the urethra, that might also be a part of the infection. Either a biopsy of the sores or cystoscopy is required to look inside the urethra which will help in reaching the diagnosis. Once the diagnosis is confirmed then treatment can be planned.
