Hello doctor,

I started mountain biking for hours per day. My knees started getting a little irritated, so I stopped. My knees hurt when I sit in a chair or biking. I tried resting them, but they just got worse. They got better when I tried physical therapy for a few months by increasing my knees' load. Then, a few weeks ago, they started getting worse again. I did an MRI and an x-ray and attached my reports.

The orthopedician gave me a Hyaluronic injection in my left knee and said there was a lack of lubricating fluid in my knees. He prescribed Etoricoxib and advised me to take rest for ten days. This treatment did not work. My knees are getting worse than before the treatment.

What should I do now?

Hiking uphill can damage the knee joint and surrounding cartilage due to compressive forces or stress on the knee during hiking, as knees absorb the impact of your body weight and the weight of your backpack. MRI of the knee joint may help determine the exact cause of your present sufferings and then treat them accordingly.

In the meantime, you can take painkillers like Ibuprofen, Naproxen, or Aceclofenac. Apply ice on your painful area, take Calcium and vitamin B complex supplements, and ensure complete rest for one month.

Thank you for joining again. It is not MRI; it is the x-ray film of your left knee joint. X-ray film shows narrowed joint space. This narrowed joint space may result from overuse of knee joint, repeatedly putting extra weight, or excess stress on knee joint from hiking, obesity or overweight, lack of sufficient synovial fluid in the space, arthritis, etc.

Ao, kindly stop hiking for few months, avoid putting extra stress on your knee joint, focus on weight reduction, and take rest to subside your knee sore. It is advisable to do an MRI of the knee joint to detect the exact cause for your present sufferings and then treat them accordingly.

Thank you for joining again. According to your MRI (magnetic resonance imaging) report, it suggests that borderline patella Alta with mild edema noted at the infrapatellar fat pad. This condition occurs most probably due to infrapatellar fat pad impingement. It is suggestive of tendinopathy at the insertion of the medial head of the gastrocnemius with a possible partial tear of the fibers.

Fat pad impingement may result from impinging the infrapatellar fat pad between the femoral condyle of the thigh and patella due to minor injuries. This condition may cause signs of inflammation, pain, or swelling on the affected knee.

To treat fat pad impingement, you can apply cold compresses on the affected knee joint several times a day. Some exercises like stretch exercises and strength building may be helpful to cure your infrapatellar fat pad. In severe pain, you can take painkillers like Ibuprofen, Naproxen, or Aceclofenac to ease your soreness.

Again, according to your MRI report, there can be a presence of tendinopathy that may result from the breakdown of the tendon's collagen due to minor trauma or repetitive overuse of the tendon. Applying cold compresses on the affected area several times daily, performing a range of motion exercises, taking rest, consuming painkillers may help cure tendinopathy gradually. It may take three to six months for subsiding tendinopathy.

There can also be a presence of partial tear fibers due to minor trauma. Usually, partial tear fibers do not need treatment, and they heal overtime on their own. Rest with or without taking painkillers may help to subside the complications. But it may take two to three weeks to five to six months for a full recovery.
