Is a desire to have a mental illness a disorder by itself?

Hello doctor,

I have something which affects me a lot. Basically, I desire to have a mental illness, and I have been even faking certain symptoms. To be exact, I do actually have mental health problems, I am diagnosed with Asperger’s, bipolar disorder, and OCD. However, I have a feeling I am not ill enough, and I want to be much sicker than I am right now. I know that is not normal and I am incredibly ashamed of it. Right now I am in remission of BAP, and I should be glad for that, but instead of it, I am desperate and unhappy. The only thing I want is a relapse and it is not just a wish, I am doing something for it. I go to sleep at different times every day to destroy my sleeping rhythm, and I do not take my meds (Valproate for BPD) regularly. I always skip them on purpose in such a way that nobody can notice, I am afraid to stop taking them at all because of the potential reaction of my parents. Then there are things in which I just pretend to look sick, for example, I tell people that I have bad dreams and sleep paralyzes frequently, while I almost do not. When I was hospitalized with mixed mania and then with depression this year, I felt a strong need to be the sickest person in the psych ward or at least look like that. I made certain symptoms like hallucinations up. I also exaggerated everything. I had severe insomnia but I always purposely slept even less, I had meltdowns but some of them I triggered purposely or I just made them up. I self-harmed to look ill, not because of an inner need. I do not even know how sick I really was and what I had just faked. I have heard about the factitious disorder and I wonder if I have it or not. I am not sure because I have a feeling it does not fit perfectly with me. Is a strong desire to have an illness a part of that disorder? Please, if you have an idea what is wrong with me, tell me. I am disgusted by myself, I want to stop this desire, faking and lying but it is just impossible. I am afraid to tell my real-life psychiatrist because he would not ever believe me on any real symptom then. I am also too ashamed to tell anyone I know. I am sorry this message is too long.

Will in-patient treatment for young adults help for ADHD and social phobia?

Hi doctor,

I am 20 years old and I just started taking online college classes. I attempted to go to in-person classes but I had panic attacks and I dropped out. For the last two years, I only leave my apartment to go to the movie theater and grocery store. I also go to the doctors and therapists, but recently my mom and I made the decision to stop trying to find the right fit for therapist and just focus on school. I also am having problems going to my psychiatrist now. I live with my mom right now, but I would love to change that in the future. I do not know how to work towards that goal though. I was wondering if going to a two month in patient treatment facility for young adults would help? My official diagnoses are ADHD, social phobia, and depression. I also do not have any friends and sometimes have trouble feeling safe outside my room. (I am not actually in any danger, I just feel like I am).

What causes mood swings from excited to depressed state?

Hello doctor,

I have these big changes in my mood and activity levels. I will swing from feelings of euphoria and lots of energy during this time. I am extremely aggitated, my thoughts race and I feel as if I do not need much sleep. But I will then crash to an extremely depressive state where I will binge eat, I loose all my motivation. I cannot think or make decisions. I also am often very anxious and I have bursts of extreme anger.

What could be the reason for fainting episodes of someone with a childhood trauma?

My friend who is about to face an exam in science has some issues because of an experience in childhood. The experience is as follows.

When she was small and was alone at home, she witnessed a woman meeting with an accident, and lying in a pool of blood. It made her dizzy and she passed out in the bathroom. She woke up later, still alone. Now, whenever she sees blood or even for no specific reason, she remembers that event and then she passes out. She told me that it happens frequently when she is learning or reading biology. She has told her parents but they haven’t taken any action about it. I intend to talk to them about this. Can you please tell me what is happening to her? Whom should she consult and how may I be of any help to her? Kindly help.

What is the dosage of Dapoxetine for premature ejaculation?

Hello doctor,

I am 45 years and suffering from severe premature ejaculation (cannot sustain for a minute or two) and mild to moderate erectile dysfunction (less hardness and short length after erection, difficult penetrating other than the missionary position, long delay in repeat erection) since the beginning of my sexual life. For last few years, I am taking Dapoxetine 30 mg (Duralast 30) before sex. It gives me a bit control but as I find 30 mg not much effective so I started taking two pills of 30 mg sometimes together and sometimes after a gap of 3 to 6 hours. Occasionally, I also add 25 mg Sildenafil or Tadalafil 20 mg (Tazzle) for improving erection. My problems are as under:

  1. I find Dapoxetine 30 mg not effective but when I take two tablets, can control the ejaculation in a better way but has nausea feeling and low libido. I also feel too hungry after 1 to 2 hours of taking tablets. Besides even two tablets has effect for 2 to 3 hours and does not remain effective if sex is delayed. Now I started giving a gap of 2 to 3 hours between two doses of 30 mg which reduced the side effects. Is this pattern of dose fine?
  2. Is there a probability of any longterm effect of taking 60 mg daily dose of Dapoxetine?
  3. As most of the time one cannot fix the time of sex in advance as it depend upon the mood of the partner too. So Dapoxetinecannot be used as a curative medicine to improve the condition permanently or it is only SOS?
  4. How can I reduce the side effect especially of low libido and nausea?
  5. Effect of sildenafil remain a bit longer and help in erection even for two days of taking the tablet. What is your recommendation of doses? How to enhance the length of the penis as my penis face difficulty in reaching partner’s G spot and it reaches to full length only just before erection. Please advise a remedy.
