Hi doctor,

I have a strange feeling in my left testicle. I ordered a vacuum penile pump, and I used it only for two days. After that, I threw it away fearing an injury, as it turned my urine red. After five days, I went to a doctor, because I had a strange feeling in my testes, but I did not have pain. The doctor gave me tablet Diclofenac 100 mg and Cystone tablet. I started taking these tablets, but after two days, I have pain in my left testis and pubic area. I also had pain on the base of my penis and erection problem. So, I went back to the same doctor, who gave me Paracetamol and Nimesulide. I got a little relief, but after a month, I still have little pain. My testes are red, and I have pain in my pubic bone area. My erection is normal now. Is this because of the injury from the penile pump? I am attaching my ultrasound report. What should I do? Please help.

Thanks for your query. I have seen the attached ultrasound (attachment removed to protect patient identity).

  • Your ultrasound suggests that you have varicocele, which is the cause of pain and discomfort and other symptoms in your testes and base of the penis.
  • In varicocele, the venous valve in and around the testis becomes incompetent or weak, and the blood gets accumulated in these veins, which get engorged. The pain is due to stretching of the nerve endings in walls of the veins due to this.
  • If you have discomfort and not severe pain, then it can be managed by taking painkillers and using scrotal support, so that your scrotum do not hang down. For severe pain, surgery is the treatment.
