Hello doctor,

I am 37 years old. My random blood sugar level is 227 today, but I had a sweet tea 15 to 20 minutes before getting the test. Should I test it again? Can I still control my diabetes, or does it have no treatment?

I went through your post. You have not mentioned whether you are on any treatment for your diabetes. However, I would suggest a few things.

Diabetes is a treatable disease, and people lead a normal life despite being diabetic, but they still need treatment. If you are not on any treatment, I suggest you start Metformin 500 mg at night. Consult your specialist doctor, discuss with him/her and take the medicines with their consent.

If you are already on medications for diabetes, a mild increase in the dosage will be needed; however, for that, you will have to mention your medications.

Start regular exercise. A brisk walk for at least 40 minutes a day would do for at least five days a week.

Do your kidney function test, liver function test, HbA1c, and a spot urine examination for any diabetes-related problem.

I have read your query and I am sorry for such discomfort. ESR (erythrocyte sedimentation rate) is a non-specific test that does not precisely help to locate the cause or problem in the body rather it gives clue that some kind of inflammation or infection is present in the body. Just based on the ESR value the list of differential diagnoses would be huge. So the test is coupled with other relevant tests depending upon the patient\'s description of the medical history to rule out or confirm possible causes of high ESR value. You have mentioned chronic pain, fatigue, and weakness in the query. It points towards some autoimmune condition based on the medical history you gave but you need to do a few tests to confirm or rule out any ongoing autoimmune condition or disease in the body. I would suggest you the following:

1) CBC (complete blood count).

2) Autoimmune panel test including ANA (anti-nuclear antibody), RA (rheumatoid factor) factor, CRP (C-reactive protein) test.

3) Metabolic panel test including blood sugar(HbA1c), LDL (low-density lipoprotein), HDL (high-density lipoprotein), total cholesterol, triglyceride.
