Medical Case Details:

An 81-year-old edentulous patient has come to the clinic with a history of Osteomyelitis so doctors told her not to place implants anywhere or anytime. So she refused to do so. But she wanted a denture with no palate extension, for a complete denture and she did not have a bone in the left lower back region. What kind of procedure can you suggest?

    Bone Grafting: If the patient is willing to reconsider surgical options, bone grafting might be a solution to address the lack of bone in the left lower back region. This procedure would involve using bone from another part of the body or a synthetic bone substitute to build up the deficient area. However, given her history of osteomyelitis, this option may need careful consideration and consultation with her medical team to assess the risks.

    Removable Partial Denture (RPD): For the lower arch where there is a lack of bone, a removable partial denture can be fabricated. This type of denture can be designed to avoid putting pressure on the deficient bone area and can be customized for comfort and function.

    Implant-Retained Overdenture: If the patient is open to implants in areas where bone is sufficient and there is no risk of osteomyelitis recurrence, an implant-retained overdenture could be a solution. This type of denture can have minimal palate coverage and provides better stability and retention. However, careful assessment and planning are essential to ensure implant placement in safe areas.
