A 6 months female body comes to my clinic with her parents by the following complaints:
1. Red cherries like blisters over her abdominal areas since 10 days.
2. It's spreading continuously.
3. Parents don't complain about fever, pain or itching or any abnormal behaviour by child.
4. She already took antibiotics and antifungal treatment oral and topical but no results.
I request to share your expert opinions and valuable inputs on this case.
Thanks in advance.
Considering the age of the child (6 months), morphology described as red cherry-like lesions, progressive increase over a short period, absence of fever, pain, itching, or systemic symptoms, and lack of response to both topical and systemic antibiotics and antifungals, an infectious etiology is unlikely.
The overall clinical picture is most consistent with a benign vascular lesion, most likely **superficial infantile hemangioma** in the proliferative phase. This age group, asymptomatic course, progressive spread, and characteristic appearance strongly support this diagnosis.
Other differentials such as bacterial or fungal infections, vesiculobullous disorders, or inflammatory dermatoses are less likely given the absence of local inflammation, constitutional symptoms, and poor response to antimicrobials.
In such cases, management is usually conservative. Observation alone is often sufficient, as many lesions stabilize and eventually regress with time. When intervention is considered due to continued proliferation, parental concern, or cosmetic reasons, topical beta-blockers such as **timolol** have shown good efficacy in superficial hemangiomas with minimal systemic absorption when used appropriately. Systemic therapy is generally reserved for complicated or function-threatening lesions.
Overall, this appears to be a benign vascular condition rather than an infectious process, and further antimicrobial therapy would not be beneficial. Regular monitoring of lesion size and progression would be appropriate.
