The patient is having a complaint of some discharge from the perianal region without pain from six months.

Hello Dr Ashish.
Discharges from perianal region without pain is the classical feature of Fistula in ano. You should examin it physically and per rectum. Probe to know it track. The best treatment of fistula in ano is Ksharasutra ligation.

Thats right. Only after a per rectal examination, we can know what the condition is. Chances are it is a rectal pathology. Always better to cross check.

It may be an anal fistula. A physical examination of this area with a manual rectal examination is necessary. As there are no other symptoms, it could be an anal fistula.

From the complaints it looks like it is most likely a Fistula in Ano. Many times, it has been noted that a Chronic Perianal Abscess might be co-existing with the Fistula in Another. A per rectal examination is a must. A probe must be also passed to properly delineate the opening of the Fistula. Surgery is the best solution to this problem. Fistulectomy or Lay Open Fistula tract is done and then the wound heals by secondary intention
