In this case the back wall(distal surface) also seems to be involved (class 2 caries)

Depth of this caries can be detected by an intra oral periapical radiograph

As the patient is not complaining about any other symptom, ask not to ignore this. Negligence may lead to deep caries that may be painful and the back wall of tooth may also fracture

Sir KRESOTUM Q may be effective for prevention of caries and initial caries but in deep caries restoration is advisable

Yes, for deep caries, there is need of drilling and removal of caries is needed. Now this case is an initial stage, so only gurgling of Kreosotum Q is enough.
Hecla lava 3x powder also helpful to remove caries teeth.

This is initial caries along with stains and there are calculus deposits too (seems severe in anterior teeth- lingual side). Scaling and filling will be required. For the molar (36), based on the depth of the cavity, you can choose the restorative material. Advise patient to maintain oral hygiene.
