Management of a case of adenomyosis

Medical Case Details:

Patient age 28 yrs, complaining of pain in lower abdomen since one year. At that time usg was done and showed adenomyosis and small 1 cm cyst in one ovary. Some doctor started her on endoreg thinking of endometriosis. Along with it started on tablet Crina (northisterone) in last 10 days every month. She improved in her pain but had irregular spotting in between. She also has a history of using some vibrator over her private parts at the very start of complaint last year. There was no significant finding on local examination. Since 2 months she is continuously having bleeding despite taking continuous progesterone regularly.

Now repeat scan is showing mild adenomyosis, no adnexal cyst, and rest everything normal. Urine analysis normal. Local examination normal. After stopping endoreg and Crina her bleeding stopped after few days but pain restarted.

My question is, what is the probable diagnosis as adenomyosis should not cause this type of pain? And how to proceed with management? Thanks

The latest treatment for adenomyosis and endometriosis is Dienogest ,that is a fourth generation progesterone ,in the brand name of Endoreg .
one of the recognized complications of continuous progesterone is break through bleeding that has happened to her.

If the condition had improved with Progesterone and again restarted after cessation of drug , it's probably endometriosis of pelvis.

For being sure one vhas to do a diagnostic Laparoscopy and inspection of the pelvis.

Else ,one can be started empirically on oral contraceptive pills for three to six cycles

In endometriosis I've seen. does she have history of any surgery?
post op adhesions can also cause this chronic pelvic pain

No, she doesn't have history of any surgery. That's what my point was that it happens in endometriosis but not in adenomyosis. And there is no finding suggestive of endometriosis.
