Hello doctor,good evening
skin tag on the base of the penis are genital warts which are growths caused by STD, so here you need to check for diagnostic tests for STD. This may give clue for the problem. Mainly thesr tags are caused by HPV VIRUS.
Skin tags are grown from the surface of the skin and attached with narrow stalk where as genetal warts are lay flat on the skin surface looked like raised bumps.

First of all take the detailed history of the patient like Sexual history, any other skin diseases suffered, or any medication taken, what is his family history,
Get the blood investigation for Sexually transmitted diseases like VDRL, HPV etc...
And treat accordingly..

First of all take detailed History, is it a single skin tag or he have more than 1 skin tag . Past History of multiple sex partner should be asked . It may be a viral disease. Some are treatable very easily the right test and and course of medicine is advised . Advise him for not any sexual contact till he cures .
