Two manoeuvres will be particularly useful
1.Valsalva manoeuvre performed by exhaling against a closed airway, done by closing one's mouth, pinching one's nose shut while pressing out as if blowing up a balloon.
2. Tonbyee manoeuvre
by pinching the nose and swallowing.
3.alternating Valsalva with Tonbyee manoeuvre

Eustachian tube edema or blockage.Tympanic membrane retraction also can be there. So Otoscopy ear examination should be done. Valsalva manouver helpful. Steam inhalation also advisable. Meanwhile decongestant can be continued.

Just sharing my personal experience.... Few months back I suffered from similar problem, I tried all the techniques mentioned above but nothing worked... At last I just used the Johnson's earbuds...it worked very well. It opened up blocked ear in just a few minutes. But kindly note, it works after taking steam inhalation and after performing valsalva maneuver.

First of all after traveling through flight , his Eustachian tube got blocked and there might be fluid behind the ear which is causing pain and reduced hearing . The main thing to do is to check for any fluid behind the ear drum , Get a audio gram and tympanogram done and can confirm that .Secondly start with a week course of antibiotics, methylprednisolobe 16mg in a tapered dose ,nasal spray , and an antacid along with antihistamine for a week and examine the patient back . If he still has fluid behind the ear drum , post him for tympanotomy and advise him to continue with nasal spray for two to three months to keep Eustachian tube opened .
