Hello doctor,

Despite normal blood test reports, ECG (electrocardiogram), and EF (ejection fraction), which is 60 %. I have a high pulse, high blood pressure, fear, and anxiety. I also have shortness of breath, weakness, mind numbness, and inability to sleep correctly sometimes. I am currently taking tablets Telmisartan 20 mg and Clonazepam 0.5 mg. Please help.

I understand your concern.

Your echocardiography is normal and your symptoms are not related to your heart. They are non-specific symptoms. If you have high blood pressure readings, I recommend checking your blood pressure daily for one week to confirm. Check it two times in the morning and two times in the evening, five minutes apart each time. That means four readings daily for seven days or 28 times. If your average blood pressure is high, then you will be diagnosed with hypertension. Then, we will look for the cause. Do daily exercise for 30 minutes of moderate-intensity aerobic exercise, 15 minutes of high-intensity aerobic exercise, or a combination of both. Also, do resistance exercises twice a week to increase your muscle strength. Consume less salt. Exercise will also help you if you have any anxiety issues.

As you recommended, I will do the exercise and note my blood pressure. What could be the reason behind my initial shortness of breath for a few seconds, followed by mind numbness and a high pulse? That is one of my main problems.

No pulmonary or metabolic cause could cause shortness of breath for a few seconds. There may be just a feeling of shortness of breath that most commonly occurs in anxiety. It can also happen in cardiac rhythm abnormalities like PVCs (premature ventricular contractions), but that is very unlikely in your case as it occurs in those with known cardiac disease and risk factors. If this happens again, then you may need further assessment.

I am experiencing high blood pressure again, despite my efforts with exercise. I felt better for a while, but now the issue has returned. My blood pressure is consistently high now, over 120/85 mm Hg. I do not notice it much during the day, but it becomes problematic at night, disrupting my sleep. I have been taking half a Clonazepam 0.5 tablet for the past three days, which I know I should not do, as it does not help lower my blood pressure. Yesterday, I checked my blood pressure shortly after waking up, and it was 123/80 mm Hg, which is abnormal from what I understand. Once I try to sleep, I become fixated on my heartbeat, which feels quite intense. Please recommend some medication and tests and explain why this might happen to me. Please help.

Your blood pressure readings are normal. Understand that blood pressure is classified according to levels, and at each stage, treatment is different.120/80 mm Hg is classified as elevated, but no treatment is required. 130/89 mm Hg is called stage 1, and treatment is only advised if there is any other risk factor like diabetes or raised cholesterol levels 140/90 mm Hg or more is called stage 2. Medicines are advised if average blood pressure remains at this level or above. So, in your case, no medicines are needed. Exercise and a low-sodium diet like a dash or Mediterranean-type diet are strongly recommended. Anxiety itself will raise blood pressure.

Also, which of the following meats should I avoid mutton, chicken, or fish? I should cut out mutton from my diet. If the problem continues, should I consider seeing a psychiatrist?

Your blood pressure is normal on these medicines, so continue these. You may eat all of these. If your lipid profile is normal, you may eat mutton, too. I usually do not recommend psychiatrists to young people who are mentally sound and can manage their stress, anxiety, and depression by themselves because psychiatrists start high-dose anti-depressants and antipsychotics, and a person becomes dependent on these. Only for those patients who need a psychiatrist or psychologist's help, I refer only those.

I underwent a lipid profile test, and here are the results.

Serum cholesterol was 148 mg/dL, triglycerides was 110, HDL was 42 mg/dL, VLDL mg/dL, and LDL was 84 mg/dL. I took the test in the morning while fasting. These results are primarily within normal ranges. How can I lower my LDL and triglycerides and raise my HDL levels? Additionally, I researched and found evidence suggesting that younger males may have a slightly increased risk of myocarditis, a type of heart inflammation, after receiving the second dose of certain COVID-19 vaccines (particularly Pfizer). My first vaccine was COVAXIN, and my second dose was likely COVAXIN. I have noticed an irregular heart rate, which began this year. Could this be related to the vaccine? I am eager to hear an expert's opinion on this matter.

Your lipid profile is normal. Please continue healthy diet and lifestyle. The certain covid vaccines did cause Myocarditis but that is self-limiting and not severe. Tachycardia may remain for up to an year but eventually normalize without long term sequela.

My sister has been suffering from tachycardia. Her pulse is never below 100 beats per minute, but her blood pressure is normal, ranging from 90/60 mm Hg to 110/70 mm Hg. My BP (blood pressure) and pulse are normal now. However, I still almost daily have heart palpitations, dizziness, and slight breathlessness. It is most apparent during the night. I sometimes feel a slight current in my heart, followed by an increase in pulse for a few seconds. I exercise daily, but I also spend a lot of time sitting in sedentary positions on my laptop as I study and freelance. Do I have arrhythmias? I am afraid. If so, what can I do to have a regular cardio system? Please recommend medicine or tests if possible. Kindly help.

No need to worry. It can be supraventricular tachycardia. It can be some other rhythm abnormality. Get an ECG done when you have palpitations. Assess regularity and count heart rate. 60 to 100 per minute is normal heart rate at rest. The rhythm should be regular.

The result concluded sinus tachycardia and anterior T wave abnormality are borderline for age and gender. While the ECG was happening, I was in panic mode with a high heart rate.
