I have had a high blood pressure of around 150/100 mm of Hg and a heart rate of 90 to 100 for four years when measured on a digital monitor. I have started using tablet Cilnidipine 10 mg once daily, tablet Telmisartan 40 mg once daily, and tablet Met XL 50 mg once daily for the last three to four days. After using the above medicines, my blood pressure reading was 130/90 mm of Hg, sometimes reaching 115/84 mm of Hg. In the morning, the first reading was 130/90 mm of Hg or 130/99 mm of Hg, with a heart rate ranging from 75 to sometimes 120. I also have intermittent dizziness and mild discomfort. I got my CBC, lipid profile, RFT, TSH, FBS, and HbA1C. My fasting blood sugar was 110 mg/dL; my total cholesterol was 229 mg/dL and non-HDL was 185 mg/dL. The triglyceride levels were normal. Do I need to start tablet Rosuvastatin 10 mg? Kindly advise.

I understand your concern. Your lipid profile is deranged; you are also a prediabetic (FBS and HbA1C range) and a known hypertensive. I suggest you exercise regularly, follow a diet modification for six months, and lose weight. I also suggest you get a renal doppler to rule out renal artery stenosis and repeat RFT (renal function test), lipid panel tests, FBS (fasting blood sugar), PPBS (post-prandial blood sugar, and HbA1C (glycated hemoglobin test). If you get a renal color doppler done and everything is fine, then the tablet Telmisartan dose can be increased to 40 mg twice daily, and tablet Cilnidipine can be stopped. I suggest you get a renal doppler done to rule out the renal cause of hypertension. You can start tablet Rosuvastatin after six months if the test results are deranged. I suggest you also exercise regularly, plan for weight loss, and follow a DASH diet (dietary approaches to stop hypertension). Even prediabetes can be managed by weight loss and exercise.
