My mother has been taking medicines for blood pressure for four months. She has been on tablet Telmipil 40 milligrams and capsule Nicardia 5. Despite taking these medicines, there is no decrease in her blood pressure levels. Instead, her BP got increased. The doctor has prescribed another medicine Arkamin to be consumed with the other two medicines. Will these medicines improve her BP? Are there any other tests needed to be done? Please suggest treatment for high BP.

I went through your query and understood your concern. The medicines prescribed to your mother are in general sufficient to control blood pressure. However, if her blood pressure is not getting controlled by these medicines, there can be two causes, White Coat Hypertension- This occurs when you get your BP checked in the doctor's chamber, and the BP is high but at home, BP is normal. This gives a false sense that blood pressure is high despite medications, Secondary Hypertension- The BP is increased due to an underlying disease such as kidney disease, renal artery stenosis, and pheochromocytoma. I suggest you monitor her BP thrice daily for one week to get a better idea of her BP control and continue taking the medicines as advised by your doctor. I hope this has helped you. Thank you for the reply. My mother had undergone certain tests like renal doppler tests, renal function tests, thyroid tests, CBC, ESR, CRP, LFT, etc. Kindly go through her reports. Also, are there any additional medicines required to cure her BP? Also, I wanted to know whether BP can be measured accurately at home. Please help.

I went through her reports (attachment removed to protect the patient's identity). However, I would also like to see the ECG (electrocardiogram) and 2-D echocardiography reports to detect the amount of pressure on the heart due to high blood pressure. Regarding BP recording, machines are not very reliable for a single-day reading which is why BP charting is recommended twice daily for one week. Kindly follow up with the reports.
