Q. I have froth in urine and facial puffiness. What is the further management?

Hello doctor,

I am 30 years old male with HTN (Telmisartan 49 mg and CTD 6.25 mg) for past six years with a body weight of 176 lbs. For some days I have noticed froth in urine and facial puffiness. I want your suggestion. My RFT was normal but I have not checked my urine for protein or USG abdomen. Can you please guide me regarding further management?

Your main concern is regarding facial puffiness. For that, you can do following investigations:

  1. Serum TSH (thyroid stimulating hormone) estimation to rule out hypothyroidism, SGPT (alanine transaminase), SGOT (aspartate transaminase).
  2. Serum protein, serum albumin estimation for checking liver disease.
  3. EKG for checking cardiac cause.
  4. If forehead pain is present and repeated allergy history, then PNS X-ray is done to rule out sinusitis.

According to cause specific treatment is given. Urine routine microscopic examination also can be done. Take care.
