I am a 69-year-old female with hypertension and severe diabetes. For the last 15 years, I used to take Telma H 40 mg, and it used to be enough, but in the last three months after having a UTI, the blood pressure is almost above 150 mm Hg; sometimes it goes to 138 but mostly above 150 or 160 mm Hg in evenings, especially after lunch. Currently, I am on Nebistar H and Cilacar. After taking Cilacar, the palpitation problem has reduced. The diastolic blood pressure and heart rates are under control, but the systolic pressure is always high. I feel uneasiness in my abdomen, and my stomach churns. The blood pressure sometimes comes down and sometimes goes up and is not even. Please suggest if I need to add any blood pressure medication to keep it consistent.

Please check your blood pressure daily a few times and make a record of it for 3 to 4 weeks to see how much is your average blood pressure. Depending upon it, medicine dosages can be modified or substituted. Daily aerobic exercise for 30 minutes or more reduces blood pressure. Weight reduction also has a good effect. Reduce sodium in the diet and increase potassium. Get a renal function test, ECG (electrocardiogram), and HbA1c (glycated hemoglobin). Also, check your urea, creatinine, and electrolyte levels.
