I have high blood pressure, leg swelling, and elevated creatine levels. I also have dizziness, and I cannot travel because of this condition.

First, I want you to tell me your previous creatinine level and an ultrasound of the kidneys. You have high blood pressure and leg swelling that point towards kidney disease, with protein in the urine. There is a possibility that you might have chronic kidney disease, which went undiagnosed. Please elaborate on your history of diabetes, previous hypertension, medication history, and family history.

Investigations to be done:

1. Urinalysis.

2. Urine protein creatinine ratio (UPCR).

3. Glycated hemoglobin (HbA1c) level.

4. Serum uric acid level.

5. Liver function test (LFT).

6. Iron profile.

7. Ultrasound of the kidneys, ureters, and bladder (USG-KUB).

Probable diagnosis:

CKD (chronic kidney disease).
