Q. My ECG shows RBBB, but no abnormality was found in echo. What to do?

Hi doctor,

I am 29 years old. I am suffering from acidity problem and hypertension. My SGPT 107, SGOT 112, bilirubin 1.1 and fasting blood sugar 83 mg/dL. All other blood test reports like lipid profile and uric acid are normal. My USG report is normal. My ECG shows RBBB and no abnormality found in the echocardiogram. I am on Atenolol 50 mg and Amlodipine 2.5 mg for the last one year.

I have seen your reports (attachment removed to protect patient identity), which are quite acceptable.

Liver enzyme up to three times of upper limit is fine. Your BMI (body mass index) also fine as it is less than 25. RBBB (right bundle branch block) is usually benign in the asymptomatic general population. The only suggestion from my side in the absence of specific query is to shift to Olmesartan instead of Atenolol. Consult your specialist doctor, discuss with him or her and get the drug altered. Atenolol has a side effect of decreased sexual performance, so I do not prescribe it in young patients. It will also cause fatigue. Moreover, it is not the first-line anti-hypertensive in any age group. For acidity, I will suggest to get an endoscopy with manometric measurement of lower esophageal junction pressure. I suggest an antacid such as Omeprazole for symptom relief.
