Hi doctor,

I feel very tired in the evenings. So, I got a medical checkup done, and I need to understand if everything is fine. Do I need to take any supplements? If yes, kindly tell me the names of the medicines. I am attaching my reports, and I am currently taking tablet Losar H and Stamlo for hypertension.

I have gone through your history, general condition, and your investigation reports (attachment removed to protect patient identity). I will need few additional information like, since when do you have your symptoms? When did you start taking the hypertension medicines? Did you gain weight recently? How is your diet? And do you workout. But, if you want me to answer, assuming that all my questions are normal, then I would suggest you the following. Weight reduction with lifestyle modification, your iron levels, MCV (mean corpuscular volume), MCH (mean corpuscular hemoglobin), MCHC (mean corpuscular hemoglobin concentration), RBC (red blood cell), and hemoglobin levels are low, implying nutrition deficiency. It is better to take Iron supplements with an iron-rich diet like green leafy vegetables, dates, etc, keep monitoring your blood pressure, get an upper gastrointestinal endoscopy, occult blood, and USG abdomen, to check for any gastrointestinal blood. Thank you for replying. I am hypertensive from the last ten years and regularly take medication, so my blood pressure is under control. I started gaining weight for ten years now, but I am trying to reduce by doing physical activity. My diet is proper. I eat both vegetarian and non-vegetarian food. I have started walking and swimming for 40 minutes daily. I shall try to include iron in my diet. I request you to suggest some supplements to improve the iron level. If even with the food, supplement, and lifestyle improvisation, the condition does not improve, then I shall go for the test.

Welcome back to icliniq.com. It is good to know that you are trying your level best to do lifestyle modifications with weight reduction. Continue the same. I suggest you take iron supplements like, tablet Ferrous fumarate or Ferrous carbonate, take one tablet per day followed by plenty of water, since it may cause some gastritis. Try to avoid taking milk, as calcium may decrease iron absorption. Consult your specialist doctor, discuss with him or her and start taking the medicines after their consent. If you do no see any improvement after a month, then consult a general surgeon regarding the poor absorption of iron, to rule out malabsorption from the gut. Thank you and take care. Regards.
